Who we are
Otley Eye Care is the data controller for information you send through this website. Contact: hello@otleyeyecare.co.uk, or write to Otley Eye Care, 26 Market Place, Otley LS21 3AQ.
What we collect
When you send the booking questionnaire, we receive the details you enter:
- your name, email address and phone number
- your town
- the service you’re interested in, and how and when you’d like to be contacted
- whether you think you may be eligible for NHS services (optional)
- anything you write in the message box (optional)
Please don’t include medical or clinical details in the questionnaire. We’ll discuss anything clinical at your appointment.
Why we use it, and our lawful basis
We use your details only to reply to your enquiry and arrange any appointment you ask for. Our lawful basis under UK data protection law is your consent, which you give by ticking the consent box, and our legitimate interest in responding to enquiries. You can withdraw consent at any time by emailing us.
Who processes it for us
- FormSubmit (formsubmit.co), a form-delivery service, passes your questionnaire answers to our email inbox.
- Our email and website providers deliver and store our email, and host this website.
These providers may process data outside the UK. Where they do, we rely on the safeguards they put in place as required by UK data protection law.
How long we keep it
We keep enquiry emails only as long as needed to deal with your enquiry and any follow-up, then delete them. If you become a patient, your clinical records are kept separately under the rules for patient records, which we explain at your appointment.
Cookies and analytics
This website doesn’t use advertising or tracking cookies. We may use privacy-friendly, cookie-free analytics that count visits without identifying you.
Your rights
You have the right to ask for a copy of your information, to have it corrected or deleted, and to object to or restrict how we use it. Email hello@otleyeyecare.co.uk. If you’re unhappy with how we’ve handled your information, you can complain to the Information Commissioner’s Office (ICO).
